Application Fee Waiver Request Forms for Financial Hardship (bring to the Advisement and Transfer Services office after completing)

• For SUNY
  Note: the SUNY application fee is waived for up to 7 colleges if you are graduating from MCC before transferring to a bachelor's degree program. There is no need to complete the form; just indicate Transfer Student Fee Waiver on the on-line application under payment options. For the SUNY Empire State application, use code: ECPTRADV (except for nursing majors). 
• For Private and Out-of-State Colleges

• College Report Request Form (bring to Registration & Records office along with the College Report Form)

Reverse Transfer Instructions (for students transferring without an MCC degree)
